I agree to waive the rights of the ownership of the Beagle once I have signed the surrender papers provided by the Beagle Club of Queensland Inc. I agree that I have fully disclosed all reasons for requesting the rehoming of the Beagle and agree that the Beagle Club of Queensland Inc. will take ownership of the Beagle once the surrender forms provided by the Beagle Club of Queensland Inc. have been signed. I agree to provide the Beagle Club of Queensland Inc. with all relevant documentation relating to the Beagle, including pedigree certificate, vaccination records, microchipping certificate, and desexing certificate. I agree to provide the Beagle Club of Queensland Inc. with proof of ownership of the Beagle and, if I am acting on behalf of the owner, provide signed documentation that I have the approval of all owners of the Beagle to surrender the Beagle on their behalf to the Beagle Club of Queensland Inc.
